Our Core Services
Long-term condition management
We offer complete end-to-end pathway management of patients with long-term, chronic conditions, such as hypertension, COPD, asthma, diabetes and lipid management. Our clinically led services incorporate digital health care solutions, such as our award-winning medical apps and online platforms, enabling effective remote patient management.
We specialise in healthcare pathways that have the greatest relevance to primary and community care settings. Over the last 40 years, we’ve successfully demonstrated how taking a pharmacist-led approach adds enormous value to the effective and efficient management of long-term conditions. That’s why our services are provided by a multi-disciplinary team of experts, led by our experienced pharmacists and based on clinical evidence.

We offer chronic disease management of high-priority patient cohorts to improve health outcomes, prevent hospital attendances and achieve compliance with standards. Managing the whole pathway, from identifying those at risk and arranging diagnostics, to treating, monitoring and managing conditions.

Document management
Our clinically led solutions incorporate digital technology to improve operational efficiency and reduce the administrative burden for clinicians, for example, managing incoming patient letters, prescription requests, pathology results or appointments.
GP practices and other care services across the country process data and receive thousands of requests, letters, results, reports and other pieces of information every day. Many of these activities don’t require clinical involvement, so we streamline the process and make things more efficient, allowing clinicians to focus only on the elements requiring their attention.
Our activities span from processing and reviewing incoming requests, to taking the appropriate action, coding and escalation based on practice/Primary Care Network specificity. Our systems also integrate with the practice’s own, e.g. Optum, making the process seamless.
Medicines management
We provide comprehensive medicines management services for primary and community care providers, from medication reviews, deprescribing and optimisation, through to reporting and ensuring compliance.
Enforcing best practice and patient safety by ensuring records are up to date, avoiding medication errors such as omissions, duplications, dosing errors or drug interactions.
Supporting dedicated workstreams around holistic medicines management needs for Primary Care.
Our pharmacist prescribers hold sessions with patients with the aim of completing the entire prescribing process in one go, without the need for GP input. This frees up GP time and maximises clinical capacity.

Living With provides healthcare providers with a single, modular, virtual rehabilitation platform which offers a catalogue of products, programmes and digital tools. Users can select from a wide range of treatable and long-term condition pathways, with each module bespoke to the specific needs, treatment and symptoms of that condition.

Squeezy is a market-leading, evidence-based, digital therapeutic App designed to improve pelvic health conditions, such as bladder or bowel incontinence. It is used by more than 700,000 patients and advocated by clinicians across the NHS and private healthcare.
There are three versions of the Squeezy app: for men, for women, and for clinicians to provide to their patients under licence. All versions are consistently among the UK’s top paid medical apps, with the female squeezy app holding the top position for more than five years.
Studies conducted in four NHS hospital Trusts show that use of the app increases adherence to pelvic floor muscle exercise programmes from 21% to 77%, with health outcomes at least 25% comparatively higher across all age groups. Missed appointments reduced by 22% on average and the annual cost savings per Trust were between £50,000 and £75,000.
