The Challenge

Healthcare demand is rising, pathways are fragmented, and clinical teams are stretched. Providers are expected to deliver more proactive, preventative care without the capacity or infrastructure to support it.

Our Approach

Formerly the Ashtons Virtual Hub, our multidisciplinary team of pharmacists, GP advisers and digital specialists delivers scalable, digitally enabled clinical capacity to practices, federations and PCNs.

The Outcome

More patients supported earlier, with consistent clinical oversight and fewer gaps in care. Reduced pressure on frontline teams, with services designed to scale safely and deliver measurable impact.

Our Services

Better Outcomes Across the Entire Care Pathway

Better Outcomes Across the Entire Care Pathway

At Agentis Healthcare, our services are designed to support the end-to-end management of long-term conditions, covering the full patient pathway from risk identification to diagnosis, treatment and ongoing care.

Our clinically led, digitally enabled solutions improve outcomes, accessibility and patient experience while reducing administrative burden and freeing up time for direct care.

By combining clinical expertise with technology, we help healthcare providers deliver better outcomes, improve efficiency and achieve cost-effective, scalable care across healthcare systems.

Our Core Services

Long-term condition management

We offer complete end-to-end pathway management of patients with long-term, chronic conditions, such as hypertension, COPD, asthma, diabetes and lipid management. Our clinically led services incorporate digital health care solutions, such as our award-winning medical apps and online platforms, enabling effective remote patient management.

We specialise in healthcare pathways that have the greatest relevance to primary and community care settings. Over the last 40 years, we’ve successfully demonstrated how taking a pharmacist-led approach adds enormous value to the effective and efficient management of long-term conditions. That’s why our services are provided by a multi-disciplinary team of experts, led by our experienced pharmacists and based on clinical evidence.

Document management

Our clinically led solutions incorporate digital technology to improve operational efficiency and reduce the administrative burden for clinicians, for example, managing incoming patient letters, prescription requests, pathology results or appointments.

GP practices and other care services across the country process data and receive thousands of requests, letters, results, reports and other pieces of information every day. Many of these activities don’t require clinical involvement, so we streamline the process and make things more efficient, allowing clinicians to focus only on the elements requiring their attention.

Our activities span from processing and reviewing incoming requests, to taking the appropriate action, coding and escalation based on practice/Primary Care Network specificity. Our systems also integrate with the practice’s own, e.g. Optum, making the process seamless.

Medicines management

We provide comprehensive medicines management services for primary and community care providers, from medication reviews, deprescribing and optimisation, through to reporting and ensuring compliance.

Enforcing best practice and patient safety by ensuring records are up to date, avoiding medication errors such as omissions, duplications, dosing errors or drug interactions.

Supporting dedicated workstreams around holistic medicines management needs for Primary Care.

Our pharmacist prescribers hold sessions with patients with the aim of completing the entire prescribing process in one go, without the need for GP input. This frees up GP time and maximises clinical capacity.

Digital health care and remote patient management

Digital health care and remote patient management

Our digital health solutions are designed to support healthcare professionals and their patients in various care settings. They enable the effective remote assessment, management and rehabilitation of greater numbers of patients, whilst requiring less clinical time and resource.

Studies show that using our digital programmes and resources results in: better patient outcomes and experience, faster recovery times for treatable conditions, increased patient engagement in treatment plans and health management, reduced clinic waiting times, significant cost and time savings for healthcare providers.

Frequently asked Questions

Who is Agentis and what experience do they bring to Primary Care?

Previously known as Ashtons, we are a leading provider of digital-first medicines management solutions with nearly 40 years of experience supporting over 300 healthcare facilities, including 80% of independent mental health hospitals in the UK. We specialise in medicines optimisation, patient safety, and outcome-driven care.

What is the vision for supporting Primary Care Networks and Practices?

We align with the evolving vision of Primary Care by enabling population health models, enhancing access, addressing backlogs, and embedding multidisciplinary teams virtually to reduce pressure on overstretched local services.

What is the Ashtons Virtual Hub?

The Ashtons Virtual Hub is a fully managed, digital-first clinical service that supports Primary Care Networks and Practices by providing remote clinical services via an MDT including Prescribing pharmacists, pharmacy technicians, and administrators. They support medicines management, workflow optimisation, clinical reviews, and administrative efficiency using your systems securely.

How does Agentis Healthcare tailor its services to meet the unique needs of our practice or PCN?

Every service is scoped individually in collaboration with the PCN or practice. We provide flexible contract models and adapt service delivery to local workflows, capacity gaps, and transformation priorities.

PCN’s and Practice will usually utilise a mix of services that best align with their goal or areas In need of support.

Do you offer any support services at the PCN level?

Yes, we offer a range of services tailored to the needs of Primary Care Networks. Our support includes population health initiatives, health inequality reduction projects, and proactive care models. We focus on preventative care, targeted interventions, and the delivery of measurable outcomes aligned with PCN priorities.

How do you help improve operational efficiency?

By taking over time-intensive tasks like document processing, patient bookings, and prescription handling, we free up local staff to focus on patient care. Our admin and clinical teams work collaboratively to reduce bottlenecks and improve workflow management.

Can you help if we’re limited by estate space or struggling with recruitment?

Yes. Our fully virtual model removes the need for physical space and eases recruitment pressures. We provide trained, accredited professionals ready to integrate immediately, with full continuity support and emergency cover.

Who will be part of our virtual team, and how are they managed?

Your team will include a lead pharmacist (Irfan Mirza), clinical pharmacists, pharmacy technicians, and administrators. Rota planning, cover, and performance monitoring are all managed by Ashtons, ensuring minimal disruption and high reliability.

How will we know if the service is making a difference?

We use medicines management reporting, audit tools, and KPI frameworks aligned to national/local indicators to demonstrate improvements in safety, outcomes, and efficiency. We continuously seek feedback to refine delivery.

What accreditations do we hold?

Can I use ARRS funding to pay for the service?

Yes. Our Virtual Hub roles are aligned with ARRS (Additional Roles Reimbursement Scheme) guidance. All of our pharmacists have completed or are on the CPPE Pathway, ensuring our service is ARRS eligible.

Will the virtual staff be part of our team or just external support?

They become an extension of your practice or PCN team. Each clinician is allocated to a specific practice or group of practices, attending meetings, using your systems, and engaging with your local care pathways just like an on-site colleague.

Who supervises the virtual staff and ensures quality?

We provide full clinical supervision, governance structures, and ongoing performance monitoring. You also get a dedicated account manager who ensures services are tailored to your needs.

What tech do we need to make this work?

Minimal setup is required. Our clinicians use your existing clinical systems (like EMIS or SystmOne) via secure access. We provide any necessary training or support to make integration seamless.

What is unique about your service?

We pride ourselves on the calibre of our people. Our teams are made up of highly experienced, primary care-trained clinicians who are fully employed by Ashtons—none of our pharmacists or pharmacy technicians are locums. This ensures continuity for patients and consistency for practices, which we believe is critical to delivering safe, effective care.

We have a team of over 45 clinical pharmacists with a wide range of expertise, enabling us to support both routine delivery and specialist projects. They’re supported by skilled pharmacy technicians and administrators, allowing us to provide a truly integrated, multidisciplinary service.

By using the right skill mix, we ensure that services are delivered efficiently, and tasks are appropriately delegated—maximising value while maintaining high standards of care.
